It's one adjustable size. The hook-and-loop strap opens wide and fastens snug, so it fits most adult ankles. The control panel sits on the outside of the ankle; the heel and the top of the foot stay uncovered.

Hemadyne is a wellness device, not a medical treatment. Check with your doctor before using it if you are pregnant, have a pacemaker or any implanted electronic device, have a history of blood clots or DVT, have an open wound or infection on the foot, or have reduced sensation in your feet (for example from diabetes or neuropathy), since you may not feel the heat properly. Stop using it if it causes pain or skin irritation.
